Registration form
Currently our practice is closed for new patients. However, people who live at the same address as one of our patients (family) are welcome.
After you filled out the registration form, we will contact your previous GP to transfer your medical file to us. When your medical file is transferred, please schedule an appointment with one of our doctors, to get to know eachother.
